How to EnrollTop of Page
Follow the five easy steps below. We are more than happy to help you at any point along the way. For assistance call (408) 928-5201.
- Click "Enrollment Request" link or "ENROLL NOW" button.
- Enter name and address of parent(s)/guardian(s), child's/children's name and age, and select center and alternate center.
- Enter submission date and time and select "SUBMIT". A copy will be emailed to the your email address.

- Click "Forms" and bring the completed forms to your scheduled appointment.
